your domain name of choice by going directly to InterNIC, the organisation
which assigns domain names, and if you are fortunate the domain name you
want will be available.
You may decide
to use any one of the myriad of registration services that connect to InterNIC,
and which may also offer useful services such as website hosting and design
as well as registration services.
You pay a fee
(usually between $50-$200 dollars to register the domain, depending on
the fees of the registering agency) and additional fees to InterNIC.
If this is all you pay, you have gotten away cheaply.

Two
If the one
you want (for example smith.com) is unavailable for registration, this
means someone else owns it, or, like fuck.com, it will not be registered
on the grounds that it is unsuitable.
Like any other
piece of property you want, you can either try to buy it, or you can settle
for another, less suitable, address which is not truly representative of
your needs.
The choice
is yours, based on how badly you want the name and how much the owner is
selling for.
Be
warned: usually the highest bidder wins out no matter how righteous your
claim.
